FSY Melbourne 2025 Terms and Conditions:

I authorize The Church of Jesus Christ of Latter-day Saints and its affiliated entities, including Corporation of the President of the Church of Jesus Christ of Latter-day Saints and Corporation of the Presiding Bishop of The Church of Jesus Christ of Latter-day Saints (the “Church”), to collect, process, and transfer to other countries, including the United States, my personal information, including health data, in order to evaluate eligibility and manage my participation at FSY. The information collected, including the health information, will only be used or processed to assist the Church in carrying out the functions of the Session.

I also consent that photographs and audio and video recordings may be taken of me, either as an individual or as part of a group, and I agree that such photographs and recordings may be used in Church-produced materials, including on the Internet.

If I or my youth is asked to participate in a research study or survey, I understand that participation is entirely voluntary and that I may withdraw at any point. Any contact information provided as part of a research study (for example, email address, or mobile phone number) will be used only for the purpose of contacting me about participating in future studies or surveys. I may opt out of receiving such contacts.

This authorization shall cover my participation in the Session and additional sessions in which I may later volunteer to participate. I authorize the Church to retain my information as long as I continue to be involved in the Session and understand that I may request and am responsible to provide updates to my personal information if such information changes.

I further understand and agree to the following:

•  FSY leaders will provide first aid for if needed (no other medical care is available at FSY, (other than assistance with administering medications when pre-arranged).

•  An ambulance will be called if deemed necessary and parents/guardians will be responsible for all associated ambulance or medical costs.

•  Parents/guardians need to be contactable for the duration of FSY. If youth need to leave FSY for any reason deemed appropriate by FSY leaders, parent/guardian will be responsible for having them picked up in a timely manner and at my own expense.

•  Although leaders will take all reasonable steps to ensure the safety of participants, I understand that FSY leaders are not responsible for personal loss or injury.

•  Personal items are the responsibility of the individual and the Church, it’s leaders or representatives, are not liable for loss or damage.

•  Registration will be cancelled if the participant is found to be ineligible or does not receive parental/guardian and Priesthood Leader approval.
